The Check Sheet, called simply Map in Kid Icarus: Of Myths and Monsters, is a map found only in Fortresses of Kid Icarus and Kid Icarus: Of Myths and Monsters. When obtained, a basic layout of the dungeon will appear in the menu. If the Torch and Pencil are also acquired, it allows Pit to track his location. The Check Sheet is discarded whenever a Fortress is cleared, and it can't be purchased in Stores.
In Kid Icarus, Check Sheets can be found in the open, though in Kid Icarus: Of Myths and Monsters, Maps can only be found by freeing a petrified Centurion.
