A false copy created by Medusa appears in Medusa's Final Battle as one of the midbosses along with Hewdraw and Pandora. It attacks similarly as it did in the first chapter.
Idol Description
A two headed dog (hence the name) that guards the Underworld. Medusa unleashes Twinbellows during an assault on a human city, using magic to supersize the bicephalous puppy. But, for Pit, the louder they bark, the harder they fall.

- Twinbellows appears to be take inspiration from both Orthrus, the double-headed guard dog owned by Geryon, and its brother, Cerberus, the three-headed hellhound that guarded the gates of Hades.
- Twinbellows's Japanese name appears to be a portmanteau of the words, "twin" (ツイン tsuin), and "Cerberus" (ケルベロス Keruberosu).
- Of all the returning bosses from the original NES game, Twinbellows is the only boss not to receive a speaking voice in Uprising.
- Official descriptions such as Twinbellows's Idol call the monster an "it", while in-game dialogue from Uprising refers to Twinbellows as a "he".
- Twinbellows's Idol description possibly references the size of his sprite in the original Kid Icarus (where he was around Pit's height), as it mentions that Medusa resized him when she attacked the town.
